Register for a Review

There are two methods that you can use to register for a Review.

1. Complete and print an application form for the Review. Mail this form along with your payment (a personal check or money order only) to:

2. Or, go to our store and register online. If you register online you will need to pay the regular (undiscounted) registration fee.
